WHAT YOU CAN EXPECT FROM US
You have the right to confidentiality.
- You have a right to information and answers to questions about your own health
and in particular:
- Any illness and its treatment.
- Possible alternative forms of treatment.
- Possible side effects.
- Prevention and avoidance of illness recurring.
- Any proposed investigations.
- To see your medical records, subject to limitations of the law (Data Protection
Act 1990) and to have access to those records written after 1st November 1991.
Full details of all our policies concerning data protection, disability, confidentiality
and other legalities are available from the practice manager.
WHAT WE EXPECT FROM YOU
The care of your health is a partnership between yourself and the practice
team. The success of that partnership depends on a number of factors. In particular,
we ask you to:
- Be courteous to reception staff.
- Plan your repeat prescription requests well ahead.
- Attend appointments on time and, if you need to cancel, cancel at least 24
hours in advance.
- Please only request a home visit if you are housebound or bedridden.
Data Protection
You have the right to see your medical records, subject to limitations of
the law (Data Protection Act 1990) and to have access to those records written
after 1st November 1991.
Suggestions And Complaints
The aim of the practice team is to provide expert and friendly healthcare
for your family. We welcome your constructive comments about our service. We
are always happy to look at suggestions for improvements. The practice manager
or any receptionist will be able to give you further information.
If for any reason you have a reason to complain about the treatment you receive
from the practice, please ask reception for a copy of our complaints procedure
or write to the practice manager, giving full details of the problem.
From time to time we will be carrying out a series of patient surveys. This information
is completely confidential and is designed to improve the service we offer to
you.
Chaperoning And Policy For Young People
If any patient wishes to have a chaperone present during an examination, please
ask the receptionist when booking in.
Any young person can request to see the doctor without a parent or guardian being
present. These consultations are confidential and we will respect the wishes
of the young person.
Freedom Of Information / Confidentiallity
We ask for information about you so that you can receive the best possible
care and treatment. We keep this information, together with details of your care,
to ensure that your doctor or nurse has accurate and up-to-date information.
It also may be needed if we see you again.
There are times when we have to pass on information about you to other people
such as hospitals, Social Services or the Health Authority. This is always done
confidentially or by removing your identifying details when they are not essential.
Everyone working for the NHS has a legal duty to maintain the highest level
of confidentiality about patient information.
Details regarding the Freedom of Information Act and our accompanying policies
are kept within our reception area. Please feel free to ask to see this documentation
from any of our reception team.
