Highview Medical Centre

How Do I....
Register With The Practice?

Patients registering with the surgery will be asked to complete various forms to enable us to ascertain details of your medical history. You will be invited to make an appointment for a new patient medical with the practice nurse. This is important as it enables us to record full details of your medical history, take up-to-date blood pressure readings and discuss your general health with you. You will be asked for a urine sample for testing as well as information about any family history of illness and chronic conditions. Children under five years of age do not need to have a medical but parents will be asked for details of all immunisation dates. The doctors at Highview encourage all children to have full immunisations.

All patients are registered with the practice rather than a particular doctor, but if you would like to nominate a preferred GP please inform our reception staff who will ensure that this information is included in your registration details.

It is advisable for all patients to have a check-up with the doctor at least once in every three-year period. If you would like to make an appointment for a health check please make an appointment with the receptionist.
